Branch 2 & 3 Structural Pest Control Business Registration Form

If you offer to perform, advertise for, solicit, or perform structural pest control or pest control within the County of Marin you must register your business annually with our department by completing and submitting the following form.

Submit a list of operators, field representatives, or applicators conducting or associated with work being conducted in the County. All listed names shall have appropriate and valid licenses.

There are three ways to submit the form:

1. Submit electronically: Complete and submit the form online for County processing.
2. Print and submit a completed form: Click the “Preview PDF” button at the bottom of the form to print it and submit using an alternative method.
3. Print and complete by hand: Print a blank PDF version of the form, fill it out manually, and submit it using an alternative method.
