You cannot register to vote in California if you are:
- not a US citizen;
- not a California resident;
- not at least 18 years old (16 for pre-registration);
- serving a prison term for a felony;
- currently found incompetent to vote by a court.
Registration form must be delivered in person or post marked 15 days before an election to be valid for that election:

- I am a U.S. citizen and resident of California. (If "No", you cannot register.)
- I am 18 or older. (If "No", you cannot register.)
- I am 16 or 17 and want to pre-register.

- Last (including suffix, such as Jr, Sr, III)
- Date of birth
- California driver license or ID card number
- Last four digits of Social Security Number
- US State or foreign county where you were born
- Home Address. (You cannot use a PO Box or Business address.)
- If you do not have a street address, describe where you live including cross streets, Route, N, S, E, W, etc.
- Mailing address
- Previous name
- Previous address
- Previous party preference
- All voters in California are mailed a ballot whether they request a mail ballot or not.
- If you do not select a Party or No Party/None, you will be registered as No Party/None.
- Phone number
- Language preference
- Ethnicity/race
- Your signature. (If you have multiple signatures sign all.)
- Date.
- By signing you swear under penalty of perjury the the information you provided is true and correct.
NOTE: If someone helped you fill out the form or returned the form for you, they must fill out and sign the "Did someone help you fill out or deliver this form?" section on the bottom of the form.
