A Vial of L.I.F.E. is a free way to store your medical information so first responders can easily find it.

What to know

The Vial of L.I.F.E. is a form that stores your medical information. When they see a sticker on your door or magnet on your fridge, emergency responders will know to look for the Vial of L.I.F.E. This way, you can share important medical information even if you become unconscious or can’t speak to paramedics. 

The Vial of L.I.F.E. includes: 

  • A small plastic vial
  • A magnet for your refrigerator door
  • A sticker for your front door
  • A medical information form
  • A small piece of Velcro

What to do

  1. Pick up a Vial of L.I.F.E. for free at any Marin County fire station
  2. Fill out the form. Complete your medical information as best you can. Include any medications you take, allergies you have, and any medical problems. Please write clearly so emergency responders can read your form. Make sure to sign and date the form. 
  3. Attach a recent photograph or photocopy of your driver’s license to the form. 
  4. Place the form in the vial, and put the vial on a shelf in the door of your refrigerator. 
  5. Place the magnet on the outside of the refrigerator in a prominent location. If the magnet does not stick to the door, use the self adhesive Velcro strip. Place one piece on the magnet and the other on the door.
  6. Place the Vial of L.I.F.E. sticker on or near your front door where it is clearly visible to emergency responders.
  7. Update the form whenever your medical history or medications change. Review the form at least twice per year when you change your smoke detector batteries.
